  • Date of commencement of business operations.
  • Number of employees in the enterprise.
  • Investment details in plant and machinery.
  • Main business activity of the enterprise.
  • Complete address of the plant location.
  • Social category of the applicant (General / Schedule Caste / Schedule Tribe / Other Backward Classes).
  • Physically handicapped status (Yes / No).
  • Name of enterprise (in the case of a proprietorship)
